• Conducting comprehensive evaluations of patients in both inpatient and outpatient settings to create personalized treatment plans that cater to individual needs.
• Administering physical therapy interventions focused on enhancing mobility, strength, balance, and overall functionality.
• Collaborating closely with nurses, physicians, case managers, and other rehabilitation professionals to deliver optimal patient outcomes.
• Providing essential education to patients and their families regarding therapeutic goals, discharge planning, and home exercise routines.
• Documenting evaluations, treatments, and progress in accordance with our facility protocols and regulatory standards.
• Engaging in interdisciplinary conferences and care team rounds to ensure seamless and coordinated care.

Education Requirements: A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program is required. Certifications: Must hold state licensure as a Physical Therapist, with CPR certification as preferred. Specialized certifications, such as in neurology or geriatrics, are a plus but not mandatory. Key Skills: Previous experience is strongly preferred. Expertise in treating patients with complex medical and mobility challenges. Excellent communication and teamwork abilities. Proficient in digital charting and clinical documentation systems.
