Bereavement Counselor

Join Sutter HealthOrganization: SCAH-Sutter Care at Home - BayPosition Overview: Preference will be given to candidates with experience in a Home Hospice environment. Assists with ensuring bereavement services are provided to the families/caregivers of hospice patients in accordance with the interdisciplinary plan of care and in alignment with SCAH, mission, goals and objectives. Bereavement counseling consists of counseling given to the individual's family after the individual's death. Bereavement counseling is a required hospice service, provided for a period up to one year following the patient's death.
