Per Diem/Variable shift The Clinical Pharmacist has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Upon hire: Oregon Pharmacist License. Preferred Qualifications: Acute care hospital pharmacy experience, 1 year or equivalent experience. Completion of ASHP-accredited PGY1 residency. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
