Purpose of the position
The Interim Director of Street Medicine is a non-clinical, non-provider operational leadership role responsible for the implementation, standardization, and quality oversight of Wellness Equity Alliance's end-to-end integrated Street Medicine workflow across California — from initial patient mapping and outreach through ECM enrollment, clinical encounters, behavioral health integration, and care coordination.
This role owns the operational integrity of the full care continuum, including:
Patient Mapping & Data Analytics (GIS-based hotspot identification, county epi data integration, CHW canvassing coordination)
Enhanced Care Management (ECM) workflow (referral/eligibility ? enrollment ? assessment ? care planning ? reauthorization ? graduation)
Street Medicine Primary Care encounter workflow (CHW field screening ? clinical triage ? provider assessment/plan ? referrals/follow-up)
Integrated Behavioral Health (IBH) workflow (screening ? referral ? telehealth coordination ? warm hand-off ? follow-up)
Housing Navigation integration across all of the above
RCM/Patient Intake-to-Billing workflow (registration ? insurance verification ? check-in ? financial clearance ? provider encounter ? checkout/billing readiness)
The Director will ensure these interconnected workflows — currently documented in draft form — are fully implemented, standardized across all sites, staffed appropriately, monitored for quality and fidelity, and optimized for both patient outcomes and revenue capture. This role also carries significant responsibility for program development, including grant, RFP, and RFA writing to support and expand these care models.
This position does not involve direct patient care, clinical supervision, or clinical decision-making. The Director partners closely with clinical leadership (CMO, State Operations Director, Practice Managers, BH providers, RNs/LVNs/MAs) to ensure operational systems, staffing, and documentation support the workflows outlined below.
Key Highlights
- Compensation : The compensation range for this role is $120,000 - 160,000 annually, with final compensation determined based on experience, qualifications, and role scope.
- Work Location & Expectations : This role requires daily in-person engagement at our Indio location.
- Position Type: Interim Pilot Role
- 90-Day Assignment: This full-time, non-clinical leadership role is being piloted for 90 days to evaluate the impact of centralized Street Medicine program leadership across WEA’s California service regions. Continuation, extension, or conversion to a permanent role will be evaluated based on pilot outcomes and organizational needs.

Key Responsibilities
Patient Mapping & Data-Driven Outreach Implementation
- Operationalize the patient mapping process by coordinating data analytics efforts (ArcGIS/QGIS, county epi maps on opioid overdose, 211 data, HIV/HCV/STI case data, syringe service program locations) to identify and prioritize outreach zones
- Ensure CHW canvassing schedules and encampment "scouting" activities are informed by mapping data and updated on a recurring basis
- Standardize intake of Member Information Files (MIFs) from managed care plans into the patient identification and outreach workflow
- Ensure partnership referral pipelines (community organizations, MCPs, hospitals) are operationally integrated into the mapping and outreach process
- Oversee documentation standards for outreach encounters per WEA protocol
Enhanced Care Management (ECM) Workflow Oversight
- Ensure fidelity to the 10-step ECM Inland Empire workflow: Referral & Eligibility ? Consent & Enrollment ? Initial Engagement ? Initial Assessment ? Individualized Care Plan ? Ongoing Care Management ? Monitor Progress ? Systematic Caseload Review (SCR) ? Reauthorization ? Disenrollment/Graduation
- Monitor Care Coordinator and Interdisciplinary Team caseloads, timeliness of comprehensive assessments, and care plan development against required standards (SDOH, functional needs, UTIs/6N benchmarks)
- Ensure required documentation systems (Athena, Care Coordination platforms) are properly utilized at each ECM stage
- Track core ECM measures (PHQ-9, BH/SUD screens, housing status, ED/hospitalization utilization, ADL indicators) for quality and reporting purposes
- Coordinate cross-training and role clarity between Care Coordinators, LCMs, and interdisciplinary/medical teams at each workflow stage
Street Medicine Primary Care Workflow Implementation
- Standardize the field-based clinical encounter workflow across all regions
- CHW pre-visit mapping and scheduling (1–2 hour encampment blocks)
- Completion of required screeners (PHQ-2/9, TAPS, HIV PrEP assessment tool, and the newly developed injection drug use risk addendum)
- CHW-to-clinical staff (RN/LVN/MA) triage handoff and Athena encounter documentation
- RN/LVN/MA clinical intake, vitals, and chart review for established patients
- Provider assessment and plan, including labs (POCT vs. phlebotomy per P&P), diagnostic imaging coordination, treatment/prescribing to designated pharmacies, and specialty referrals
- Scheduling of follow-up visits with CHW/RN/LVN teams
- Ensure operational readiness of supporting infrastructure at each site: preferred imaging partners, pharmacy relationships, and specialty referral directories
- Partner with clinical and regional leads to maintain an up-to-date specialty/pharmacy/imaging partnership directory across all counties
- Monitor screener completion rates, positive-screen escalation timelines (e.g., PHQ-9 =10 and TAPS =2 referral protocols), and registry documentation compliance
Integrated Behavioral Health (IBH) Workflow Oversight
- Oversee full implementation of the IBH referral-to-follow-up workflow
- CHW identification and Athena-based referral submission (with required patient/referral details)
- BH screening confirmation and PCP visit verification (including telehealth bridge coordination when PCP is unavailable)
- BH provider scheduling coordination across regional teams and provider calendars
- Warm hand-off protocol between PCP and BH provider
- Day-of-appointment protocol: reminder calls, field-based vitals/screener administration, Athena check-in, telehealth connection, and required on-site chaperone/safety presence during BH telehealth sessions
- Post-appointment follow-up scheduling, medication fulfillment tracking, and PHQ-9/GAD-7 completion monitoring
- Ensure adherence to exclusion criteria protocols (acute crisis, psychosis, mania, medical instability) and appropriate escalation to external/higher levels of care
- Monitor documentation completion by BH providers and case managers at each stage
- Track referral-to-appointment timelines and ensure triage occurs within required business-day windows
Housing Navigation Integration
- Ensure housing navigation is embedded across ECM, Street Medicine, and IBH workflows rather than operating as a siloed service
- Monitor coordination between housing navigators, Care Coordinators, and clinical teams to support whole-person care planning
Revenue Cycle Management (RCM) Workflow Optimization
- Oversee implementation of the 7-step RCM Patient Intake workflow: Patient Registration ? Appointment Scheduling ? Insurance Verification ? Patient Check-In ? Financial Clearance ? Provider Encounter ? Patient Checkout/Billing Readiness
- Ensure front desk, CHW, and clinical staff are trained on accurate real-time data entry, insurance verification protocols, and required consent/HIPAA documentation to minimize claim denials
- Partner with billing/RCM and finance teams to monitor documentation quality, encounter completeness, and financial clearance workflows across all field and clinical touchpoints
- Identify workflow breakdowns contributing to billing delays or denials and implement corrective training or process adjustments
- Ensure eligibility verification and reauthorization timelines (ECM and BH) are met to avoid reimbursement gaps
Program Development, Grants & Funding
- Partner with the Chief Development Officer to translate the documented workflows, outcomes data, and program models above into competitive grant proposals, RFP/RFA responses, and funder reports
- Support development of new program models and workflow adaptations for future expansion regions (Orange County, Alameda, Sacramento)
- Ensure grant/contract deliverables tied to ECM, IBH, housing navigation, and street medicine programming are operationally tracked and fulfilled
Standardization, Documentation & Org Design
- Finalize and formalize the organizational chart/staffing model options for street teams and ECM care teams across regions, ensuring role clarity across CHWs, LCMs, RNs/LVNs/MAs, providers, BH case managers, and Care Coordinators
- Maintain and update workflow schematics/diagrams (patient mapping, ECM, street medicine PCP, IBH, RCM) as living operational documents
- Ensure consistent use of Athena and other required documentation systems across all workflow stages and all regions
- Lead recurring training and best-practice guideline development (e.g., outreach engagement best practices, screener administration, telehealth connection protocols)
Quality Oversight & Site Visits
- Conduct recurring on-site visits across all active regions to directly observe workflow fidelity at each stage (mapping/outreach, ECM, clinical encounters, BH appointments, RCM intake)
- Identify variation across sites and drive standardization toward the documented master workflows
- Report quality findings, KPI performance, and operational risks to the CMO on a regular basis
Qualifications and Education Requirements
- Minimum 5–7 years of progressive experience in healthcare program operations, preferably in FQHC, street medicine, homeless health services, or safety-net healthcare settings
- Demonstrated experience with Medi-Cal managed care, CalAIM, ECM, and/or Community Supports program operations
- Experience with EHR systems (Athena preferred) and telehealth coordination workflows
- Experience with healthcare revenue cycle management principles and documentation-to-billing workflows (non-clinical operational understanding, not coding/billing execution)
- Demonstrated success in grant writing, RFP/RFA response development, and program development
- Experience managing multi-site or geographically dispersed teams, including CHWs, Care Coordinators, and clinical support staff
- Strong process design, workflow documentation, and systems-thinking skills
- Excellent written and verbal communication skills, particularly for grant and funder-facing materials
Preferred Skills
- Experience working with populations experiencing homelessness, harm reduction programs, or gender-affirming care service delivery
- Familiarity with HRSA, county Departments of Public Health, and California homeless services funding landscape
- Experience designing or refining clinical/operational workflow diagrams and org chart structures
- Familiarity with GIS-based or data-driven outreach mapping approaches (ArcGIS, QGIS, or equivalent)
