Outpatient Physical Therapist

This is a 13-week contract position.

Schedule

Full-time schedule: Monday, Wednesday, Friday from 7 AM to 4 PM; Tuesday and Thursday from 8 AM to 5 PM.

Job Description

The Outpatient Physical Therapist is responsible for evaluating patients and developing tailored treatment plans according to their specific needs and goals as directed by their physician. The therapist will oversee all discipline-specific therapy services for assigned patients and ensure the implementation of functional treatment programs that facilitate their return to community activities. Education will be provided to patients, families, students, and colleagues, ensuring adherence to the organization’s mission and professional standards.

Responsibilities

• Conduct comprehensive evaluations of assigned patients; develop and modify treatment plans focused on discharge goals.
• Document all care in medical records clearly and accurately, ensuring compliance with state, federal, and organizational guidelines.
• Participate in student programs, fostering a positive learning environment through supervision and evaluations.
• Create individualized professional development plans to pursue growth through continuing education and professional activities.
• Adhere to relevant state practice acts and maintain all professional licensure in good standing.
• Implement the organization’s performance improvement methodologies to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for physical therapy.
