About the Role


We are seeking a dedicated Physical Therapist to provide high-quality rehabilitation services for patients across the lifespan, from pediatric to geriatric populations. In this role, you will evaluate patient needs, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and overall functional outcomes. The ideal candidate is compassionate, clinically skilled, and committed to delivering exceptional patient-centered care.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans in collaboration with physicians and the interdisciplinary care team.
• Provide therapeutic interventions, including manual therapy, therapeutic exercise, gait training, mobility training, and the use of assistive devices to improve functional outcomes.
• Monitor patient progress, modify treatment plans as needed, and prepare discharge plans with appropriate home exercise programs and follow-up recommendations.
• Educate patients, families, and caregivers on treatment goals, rehabilitation techniques, and safe use of assistive equipment.
• Maintain accurate clinical documentation while following infection control practices, regulatory standards, and patient safety guidelines.

Requirements

• Active Colorado Physical Therapist License required.
• Current BLS (AHA) certification required.
• Strong knowledge of physical therapy evaluation, treatment planning, rehabilitation techniques, and patient care.
• Excellent communication, documentation, and interdisciplinary collaboration skills.
• Commitment to delivering high-quality, patient-centered rehabilitation services.
