CADC Substance Abuse Counselor: Case Mgmt & Groups

Advocate Health

Advocate Health in Illinois seeks a dedicated addiction case manager to assess patients and families, develop biopsycho-social summaries, and create individualized treatment plans with the care team. Coordinate input, revise plans as needed, and introduce patients to 12-step recovery. You will lead group counseling, supervise students, deliver education on addiction and recovery, participate in quality improvement activities, and collaborate on program development to enhance patient care. #J-18808-Ljbffr Advocate Health
