About the Position

Job Title: Medical Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum of 1 full year of acute care experience required License: CA state license required, pending license accepted but must be licensed upon start date Certifications: BLS, LCSW, MSW Must-Have: - Experience in acute care settings - Ability to manage a caseload of 20-23 patients - Experience in documentation and care coordination - Weekend availability required Description: Start date is as soon as possible. Floating expectation within the facility. This position does not accept first-time travelers. Job responsibilities include initial assessment within 24 hours. Coordination of discharge plan and all post-acute care needs is required. Candidates will work closely with a multidisciplinary medical team. Successful candidates will assist patients in navigating various services and resources. Experience in case management and discharge planning is essential. The role involves working with a variety of patient diagnoses and age groups. Knowledge of community resources and services is crucial. Candidates must demonstrate strong crisis intervention skills. Attendance is mandatory for every other weekend. This position requires comprehensive documentation according to the plan of care. The social worker will participate in care conferences and interdisciplinary activities. Commitment to providing quality care in a cost-effective manner is expected. All travelers are required to float between facilities as needed. Candidates must provide references covering at least one year of experience. Flexibility in scheduling may be required based on staffing needs. Travelers should expect to work during significant holidays throughout the contract.
