In-Home Nurse Practitioner or Physician Assistant Per Diem - Danville/Martinsville, VA

Overview Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations and support patients in the comfort of their own homes. Responsibilities Build trusted relationships during in‑home visits that last approximately 45 minutes. Review medication and medical history, conduct a physical evaluation, and perform any diagnostic tests ordered by the patient’s health plan (e.g., diabetic eye exams, spirometry). Use an iPad and an intuitive clinical workflow for each evaluation while not prescribing medication, ordering lab tests, or altering existing treatment regimens. Participate in virtual visits as required. Provide all Signify Health services, including Diagnostic & Preventive Services products such as spirometry and DEE. Maintain flexibility to travel locally and within licensed states and to meet patients in a variety of home environments. Work Environment The primary workplace is the patient’s private residence, which may include household pets, secondhand smoke, and potential airborne allergens. Clinicians must be able to perform duties safely in these settings. Required & Preferred Qualifications (NPs and PAs) Active, unrestricted license(s) in coverage area(s). Board certification (required). Multi‑state licenses (preferred). Ability to cover Danville and Martinsville, VA. Employment Details Anticipated Weekly Hours: 12 Time Type: Part time Pay Range: $41.85 – $90.13 per hour (base rate; actual offer may vary by experience, education, geography).
