LPN Post-Acute Care

Schedule: 12-Hour Night Shift

Assignment Details:

• Contract Length: 16 weeks
• Guaranteed Hours: Not specified

Requirements:

• Active LPN License
• IV Certification preferred for LPNs
• Minimum of 1 year of clinical experience required
• Minimum of 1 year of clinical experience in specialty required
• EPIC experience within the past 2 years required
• Dementia capable care certification preferred
• Prior travel experience required

Roles & Responsibilities:

• Provide direct patient care in a post-acute skilled nursing rehab environment.
• Administer medications and treatments as prescribed.
• Monitor patient conditions and report changes to the healthcare team.
• Assist with diagnostic tests and procedures as needed.
• Document patient care activities accurately in the electronic medical record system.
• Collaborate with interdisciplinary team members to plan and implement patient care.

Compliance / Notes:

• Must be able to work all holidays.
• Travel requirements include floating to long-term care/memory care as needed.
• Reliable transportation is necessary; public transportation is not accessible.
• Orientation is bi-weekly on Monday.
