A leading healthcare organization is seeking an experienced Hospice Liaison to support patients and families through compassionate end-of-life care coordination across hospital and SNF settings. This is a field-based role covering designated Northern Wisconsin territories with flexibility and strong team support.

Position Overview

• Position: Hospice Liaison
• Coverage Area: Northern Wisconsin
  
  • Green Bay
  • Oshkosh
  • Manitowoc
  • Sheboygan
  • Grafton
• Patient Volume: Approximately 2-3 patients per day
• First-Time Travelers: Accepted
• Experience Required: Minimum 2 years preferred
  
  • Hospice Liaison experience preferred
  • Other hospice experience also considered

Travelers will typically cover 1-2 hospitals depending on census and facility size. Coverage areas will vary by assignment day.

Role Responsibilities

• Monitor referral queues from hospitals and skilled nursing facilities
• Consult with patients and families regarding hospice services
• Perform patient assessments and provide recommendations for appropriate hospice placement
• Admit patients to hospice services in hospital settings
• Educate patients, families, and providers on hospice philosophy and benefits
• Collaborate closely with physicians and interdisciplinary teams

Patient Population

Adult patients with:

• Heart failure
• Cancer
• End-stage disease processes
• Other hospice/end-of-life diagnoses

Required Skills

Candidates should be comfortable with:

• End-of-life symptom management
  
  • Pain management
  • Shortness of breath management
• Hospice medications and provider education
• Hospice levels of care:
  
  • General inpatient
  • Routine care
  • Respite care
• Communicating and collaborating with physicians
• Educating patients and families on hospice philosophy
• EPIC charting and secure communication

Required Certifications & Requirements

• BLS Certification
• Valid Driver's License
• Active Auto Insurance
• Reliable Vehicle

Technology & Equipment

• EMR: EPIC (required)

• Laptop and phone provided

Support Structure

• Supervisor/Manager support via phone and chat
• Dedicated Hospice Liaison team
• Hospice Medical Directors available for guidance

Orientation

• New Employee Orientation (NEO)
• 40 hours of unit orientation with staff member
