Licensed Practical Nurse (LPN) - Home Health Full-Time, Direct-Hire

MLR is partnering with a respected home health provider to find a dedicated Licensed Practical Nurse (LPN) for a full-time, direct-hire position. This role is crucial for delivering compassionate, high-quality patient care directly in the comfort of patients' homes. Our client seeks a skilled LPN who is committed to providing exceptional, patient-centered care. A $7,500 relocation assistance package is available.

Key Responsibilities:

• Direct Patient Care: Administer medications, monitor vital signs, and assist with daily living activities.
• Collaboration: Work with physicians and registered nurses to implement care plans, including wound care, injections, and medical procedures.
• Documentation: Accurately document patient information in electronic health records (EHR).
• Patient Education: Educate patients and families on care plans and medication management.
• Compliance: Ensure adherence to home health regulations and advocate for patient needs.

Licensed Practical Nurse (LPN) - Home Health Qualifications:

• Licensure: Current and unrestricted LPN license in the state of South Carolina.
• Experience: Minimum of 2 years of practical nursing experience, preferably in home health or a clinical setting.
• Certifications: Basic Life Support (BLS) certification required.
• Skills: Strong clinical skills, proficiency in EHR systems, and excellent communication abilities.
• Personal Attributes: Compassionate, adaptable, detail-oriented, and comfortable working independently in a home environment.
