Nurse Case Manager II- Adult ED

Job Summary The Nurse Case Manager (NCM) II reports to the Manager of Case Management and Care Transitions and works under the guidance of the Nurse Case Manager Team Leader. This role coordinates a plan of care to achieve clinical outcomes, promotes a safe and timely discharge, and manages the plan of care throughout the care continuum to ensure efficient movement of the patient through the hospital system. Responsibilities Coordinate patient care plans and discharge activities to achieve clinical outcomes. Manage care transitions and ensure efficient movement of patients within the hospital system. Serve as a clinical expert regarding discharge planning, resource utilization, and case management. Implement and monitor plan of care throughout the patient’s care continuum. Qualifications Bachelor’s degree in Nursing from an accredited school of nursing. At least three years of experience as a Registered Nurse. Prior leadership experience preferred. One year of case management experience preferred. Case management certification preferred. Active RN licensure in the state of South Carolina or a compact state. Certification in Case Management preferred. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required. Physical Requirements Ability to stand, sit, walk, and perform tasks continuously. Ability to lift and carry up to 50 pounds unassisted. Ability to climb stairs, work in confined or cramped spaces, and reach overhead frequently. Physical endurance for repeated movements of hands, wrists, elbows, and shoulders. Good vision, hearing, and sensory function; ability to work in a latex‑safe environment. Work Hours Scheduled weekly hours: 40 hrs.
