Verify the Plan of Care before each visit and provide care according to RN delegation, physician orders, assessment data, and established standards Incorporate patient care goals into nursing visits and document them in skilled nursing notes Communicate significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and involved personnel Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician Implement patient safety plans using patient, family, and community resources Teach patients appropriate self-care techniques while delivering quality care Suggest revisions to the Plan of Care and notify the RN Case Manager Provide substantial and specialized nursing services under the direction of an RN or physician Collaborate with the RN Case Manager for advice, assistance, and instructions Document medication regimens and update medication profiles Treat patients and caregivers in an effective and compassionate manner Take actions to prevent avoidable hospitalizations and emergency-room visits Provide supportive communication to physicians, patients, families, visitors, and agency personnel Report to the Clinical Team Manager Requirements Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current state license as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance Reliable transportation to perform job tasks Core Competencies Demonstrates expertise in patient care management, including the ability to implement care plans, communicate effectively with healthcare teams, and provide compassionate support to patients and families.
