Team 3: Ashroken, Centerport, Cold Spring Harbor, Commack, Dix Hills, Eaton's Neck, Elwood, Fort Saonga, Greenlawn, Half Hollow Hills, Hauppauge, Huntington, Huntington Bay, Huntington Station, Kings Park, Lloyd Harbor, Melville, Northport, Northport East, & Smithtown.

Fee-for-Service Physical Therapists are required to:

1. Work a minimum of 2 shifts per pay period (every 2 weeks)
2. Attend the full first week of orientation, Monday through Friday from 8:30 AM 4:30 PM
3. If no prior CHHA experience, commit to training 2 days per week until fully oriented and independently managing

Job Responsibilities

Our Home Care PTs:

• Review patient charts and medical history
• Develop a personalized treatment plan
• Teach therapeutic exercises and stretches to improve patient's conditions
• Use hands-on therapy to promote healing
• Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
• Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for health promotion as directed by physician's plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications

• New York State PT license
• Minimum of one year of recent hands on experience as a Physical Therapist.
• Valid New York State Driver's License and reliable transportation

Posted Salary Range

USD $95,000.00 - USD $120,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
