Nurse Practitioner/Physician Associate (NP/PA) Clinic Setting

As a clinician with AgeRight Clinical Services, you will work on a team of dedicated Physicians, APPs, Nurse Practitioners, and Physician Associates from a variety of different specialty backgrounds to deliver compassionate, holistic care to our patients. This is a fantastic opportunity for an experienced Family, Adult, or Geriatric Nurse Practitioner or Physician Associate who desires to work in a clinic setting within a senior living community, that enjoys autonomy, and is looking to exercise their clinical skillset in our ever‑evolving environment. You will have the opportunity to build relationships with your patients and make a meaningful difference in their lives. Benefits Package may include Competitive Wages Paid Time Off 401k with company match of 3% Health, Dental, and Vision Insurance Flexible schedule, NO evening, weekend or holiday on‑call requirements $2,000 to spend on CME each year License Renewal Reimbursement Malpractice Coverage Schedule Monday‑Friday, 8a‑5p with no on‑call or after‑hours requirements. Responsibilities Conduct clinical interventional and episodic direct care visits, and appropriate re‑evaluation of care visits. Diagnose and treat common acute illness and injuries Order and interpret diagnostic tests Develop and implement treatment plans Collaborate with physicians and other healthcare professionals to ensure coordinated patient care Perform minor surgical procedures as needed (repair lacerations, I & D, etc.) Review medications, orders, and diagnostic tests, and respond to clinical abnormalities appropriately for all patients under their care, as allowed and/or requested by collaborating physicians. Engage in advanced care planning conversations Conduct Annual Wellness Visits on all senior living residents, as applicable Coordinate care with other disciplines, such as Therapists, Specialists, and Social Workers Qualifications Current, unrestricted Oregon state license, or ability to obtain an Oregon state NP or PA license and current board certification in your field Masters Degree in Nursing or Physician Assistant Studies Experience working with older adults in a clinic, medical office, or urgent care setting preferred Proficiency in medical documentation and coding Current DEA certificate EEO Statement “Be here.
