Position Overview

The role involves coordinating the utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation. This position oversees the management of acute patient populations across the care continuum, emphasizing coordinated and integrated care to prevent unnecessary admissions or readmissions.

Key Responsibilities

• Coordinate discharge planning and interventions for high-risk patients presenting to the Emergency Department.
• Promote patient wellness and improve care outcomes.
• Ensure efficient utilization of health services and minimize payment denials for patients with complex health needs.
• Manage acute patient populations effectively across various healthcare settings.

Requirements

• Basic Life Support (BLS) certification.
• Valid nursing license in California.
• Minimum of 2 years of experience in a relevant setting.
• Charge experience is required.
• Experience in managing patient ratios of 20 or more is preferred.

Preferred Experience

• Long-Term Acute Care (LTAC) experience.
• Experience in Trauma Level I or II facilities.
• Previous experience in a community hospital setting.

Skills

• Proficient in care coordination and discharge planning.
• Experience utilizing InterQual Criteria and Milliman Guidelines.
• Knowledge of appeals and denials processes.
• Familiarity with concurrent and retrospective reviews.
• Understanding of medical necessity criteria.
• Proficient in CPT coding and billing, ICD-10 coding, and healthcare regulations.
• Compliance with HIPAA, OSHA, and guidelines from the Joint Commission.

Work Schedule

This position follows a day shift schedule of 5x8 hours, from 08:30 AM to 05:00 PM, with a minimum of 2 weekend shifts required per month.

Patient Details

• Patient Age Groups: Adults and Geriatrics
• Daily Census: 100-120 patients
• Number of Beds: 120

Compliance and Onboarding Requirements

• Completion of a 7-Year Work History Verification is required.
• All compliance documents must be submitted before the stated deadline.
• Participation in modules during orientation is required, with non-billable hours factored into total time.
