Qualifications
Education: Bachelor's Degree in Social Work, Human Services, Public Administration, Psychology, Counseling, or a related field required.
Master's Degree preferred.
Credentials: Valid New York State Driver's License and access to a reliable vehicle.
Experience: • Minimum five years of progressively responsible experience in human services, case management, housing, social services, homelessness services, family support services, or a related field.
• Minimum three years of supervisory or management experience required.
• Demonstrated experience in program administration, staff supervision, quality assurance, and community collaboration.
• Experience with grant-funded programs preferred.
• Experience developing community partnerships and participating in strategic planning initiatives preferred.
Relevant years of experience and education will be considered
General Description
The Director of Case Management provides leadership, oversight, and strategic direction for the Agency's Case Management Services Department, encompassing several key programs. The position is responsible for program performance, staff supervision, quality assurance, regulatory compliance, budget management, and service outcomes. As a member of the Agency leadership team, the Director supports strategic planning, program development, community partnerships, and organizational growth initiatives. The position serves as an Agency representative in the community, participates in program planning efforts, and promotes a trauma-informed, client-centered approach to service delivery
Essential Duties and Responsibilities
Leadership and Strategic Direction
- Provides overall leadership, management, and oversight of the Agency's Case Management Services Department including:
- Permanency Resource Center
- Homeless Coordination Program
- Social Security Maximization Program
- Emergency Assistance Program
- Supervised Visitation Program
- Develops and implements strategic plans that support Agency priorities and community needs.
- Participates in Agency strategic planning, program expansion initiatives, and organizational development efforts.
- Serves as staff leadership of the Agency Program Planning Committee.
- Identifies emerging service needs and opportunities for program development and growth.
- Represents the Agency in community collaborations, coalitions, government meetings, and stakeholder forums.
- Maintains productive relationships with funders, regulatory agencies, community partners, and referral sources.
- Develops and recommends policies, procedures, and service delivery standards that enhance program effectiveness and client outcomes.
- Ensures programs operate within a trauma-informed and strengths-based service framework.
- Provides direct supervision to program supervisors and staff
- Hires, orients, coaches, mentors, and evaluates program personnel in accordance with Agency policies and procedures.
- Establishes performance expectations and conducts regular performance and development conversations.
- Develops corrective action and improvement plans when necessary.
- Ensures ongoing staff training and professional development.
- Promotes employee engagement, collaboration, teamwork, and accountability.
- Ensures appropriate staffing structures to meet program goals and service demands.
Program Administration, Service Delivery, and Financial Management
- Ensures quality service delivery and positive client outcomes across all assigned programs.
- Oversees systems for program monitoring, data collection, and outcome measurement.
- Reviews program performance metrics and utilizes data to improve services and operational efficiency.
- Ensures effective coordination and communication across Agency departments and community partners.
- Oversees utilization review, program evaluation, and continuous quality improvement activities.
- Supports the development and implementation of evidence-informed practices and innovative service models.
- Ensures compliance with contractual obligations, funding requirements, and service standards.
- Participates in special projects, research initiatives, and program enhancement activities.
- Communicates program performance, emerging trends, and operational issues to the Executive Director.
- Oversees financial management of assigned programs, including budget development, monitoring, and resource allocation.
- Reviews program revenues and expenditures and implements strategies to ensure fiscal sustainability.
- Works collaboratively with the Finance Department to prepare, monitor, and manage budgets.
- Assists with identifying funding opportunities and developing grant proposals.
- Supports fundraising activities and program sustainability efforts.
- Monitors program productivity, performance indicators, and contractual benchmarks.
Quality Assurance and Compliance
- Ensures compliance with all applicable federal, state, local, funder, and Agency requirements.
- Oversees quality assurance activities, program audits, and corrective action plans.
- Develops and maintains program performance dashboards and outcome reporting processes.
- Ensures adherence to documentation, confidentiality, billing, and reporting standards.
- Maintains awareness of changing regulations, best practices, and service delivery models.
- Participates in Agency committees, meetings, trainings, and professional development opportunities.
- Supports the Agency's Mission, Vision, Values, Strategic Plan, and Core Competencies.
- Performs other duties as assigned
