Full-Time Physician – Post-Acute Care Full-Time Physician – Post-Acute Care Location: Southern, NJ | Schedule : Full Time We are seeking a compassionate and skilled full-time Post-Acute Care Physician to join our care team. In this role, you will support patients and families as they transition from hospital to post-acute settings, overseeing recovery and managing complex medical needs with clinical expertise and empathy. About The Role The Physician will provide comprehensive medical evaluation and management for patients in post-acute settings, including skilled nursing and rehabilitation facilities. Responsibilities include: Leading and coordinating post-acute medical care plans Managing acute and chronic conditions during recovery Providing symptom management and goals-of-care discussions Collaborating with interdisciplinary teams, including nursing, therapy, and case management Supporting safe transitions of care and reducing readmissions This role offers the opportunity to build meaningful relationships with patients and families while guiding them through critical stages of recovery. The actual compensation package could vary based on factors such as, but not limited to, the applicant’s experience, internal equity, and alignment with market data.
