Physical Therapist (PT) – PRN Autumn Lake at Greenfield Milwaukee, WI $55–$65/hour Join Autumn Lake at Greenfield as a PRN Physical Therapist (PT) and provide individualized rehabilitation services in a collaborative skilled nursing and post-acute care environment. Position Responsibilities Perform physical therapy evaluations and establish individualized plans of care Provide skilled interventions focused on strength, balance, gait, mobility, and functional performance Monitor resident outcomes and modify treatment plans as appropriate Educate residents and caregivers on mobility and safety strategies Work closely with PTAs, nursing, and other rehabilitation professionals Maintain timely clinical documentation Qualifications Active Wisconsin Physical Therapist license required Strong clinical assessment and communication skills Skilled nursing or post-acute care experience is a plus Flexible PRN hours $55–$65/hour Collaborative therapy environment Direct Hire.
