Responsibilities include home visits, intake and assessment (Family Needs Assessment, individualized service plans), collaboration with medical providers and community partners, case monitoring, documentation, and program reporting. Duties may include supervision of interns, training peers, and participating in quality assurance. May work under ICS during public health emergencies and must comply with applicable policies and procedures. Travel statewide and out-of-state to perform essential functions is required. Essential Job Functions (EJFs): Attend work regularly and perform duties as assigned. In addition to basic CM duties, perform program-area specific CM duties across multiple CM programs, including Medicaid Case Management for CPW, PCS and CFC, CSHCN-SP, Birth Defects Epidemiology and Surveillance (BDES), Safe Sleep Ambassador, Newborn Screening, and Public Health CM. Conduct presentations, provide training and technical assistance, and engage with community organizations to improve health outcomes. May supervise interns and participate in quality assurance reviews. Travel and on-call duties may be required to respond to public health emergencies. Knowledge, Skills and Abilities (KSAs) Knowledge: Social work and case management principles; programs such as Medicaid, CSHCN, CPW, CPS/Foster Care, SSI, NBS/NBHS, Lead, ECI, CHIP; QA and TA policies; community resources; cultural and socioeconomic health and disability considerations. Skill: Use of email, internet, databases, Microsoft Office; conducting intake and family needs assessments; developing service plans; conflict resolution, planning, prioritizing, organizing; providing training and TA to professionals; decision making. Ability: Communicate effectively in writing and verbally; assess and synthesize complex needs; build relationships across disciplines; interpret policies; submit timely reports; maintain confidentiality; manage multiple workloads; follow policies; transport independently; lift up to 30 pounds. 1. Current, unrestricted social work license issued by the Texas State Board of Social Worker Examiners
2. Valid Class C Texas Driver's License or equivalent
