Emergency Palliative Care Nurse Navigator

Rutland Regional Medical Center in Rutland, VT seeks a Palliative Care Nurse Navigator to support the Emergency Department. You will guide patients and families through serious illnesses, coordinate care, triage referrals, and facilitate goals-of-care discussions. Requirements include nursing degree (BSN preferred), 3 years of nursing experience, and 2 years in palliative or hospice care preferred.
