ELIGIBILITY CRITERIA FOR INITIAL APPOINTMENT:
Physicians trained in Cardiology fellowship programs may have variable levels of expertise in a variety of different areas, depending on the focus of their particular training experience. The American College of Cardiology (ACC) has defined the education, training, and experience necessary for many of these areas. The ACC 2008 Recommendations for Training in Adult Cardiovascular Medicine Core Cardiology Training (COCATS3) (Revision of the 2002 COCATS Training Statement) published January 22, 2008 are available at and was used as the foundation for the privileges in Cardiology. Additional information was obtained from the ACGME Program Requirements for Fellowship Education available at and the Clinical Privilege White paper on Clinical Cardiac Electrophysiology. Additional resources were used for procedures that cross specialty lines. Successful completion of a hospital affiliated Internal Medicine residency program of at least 3 years' duration, and completion of a fellowship in Cardiology of at least 3 years' duration accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada. Clinical Cardiac Electrophysiology requires successful completion of an approved ACGME training program that is 12 months in length and is in addition to the Cardiology fellowship. The Credentials Committee; Physician Executive or Medical Affairs Committee; and the Board of Directors or the Board of Governors may consider training at residency and fellowship programs accredited by organizations other than the above. Certification by the appropriate specialty /subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA), or the Royal College of Physicians and Surgeons of Canada, as applicable. Those applicants who are not board certified at the time of application but who have completed their fellowship training within the last five years shall be eligible, however must achieve board certification within five years from the date of completion of their fellowship training unless the appropriate governing Board waives such requirement. (Refer to each entities policy for grandfather exceptions) Level 1 (Basic Cardiology) -- Basic training required of all trainees to be competent consultant cardiologists. Provider is competent to manage brady arrhythmias and tachyarrhythmias. This level is addressed in the Cardiology Core Definition of clinical privileges and procedures. Level 2 (Advanced training in management of complex cardiac arrhythmias) - Providers are experienced in programming and follow-up management of all types of bradycardia pacing systems. Provider has advance expertise in temporary pacing, Cardioversion, and complex arrhythmia electrocardiogram (ECG) interpretation. Level 2 training will not qualify the provider to perform the invasive surgical procedures required to implant arrhythmia control devices or pacemakers unless HRS guidelines are met (see link below for specific information).
See Special request for pacemakers. Level 3 (Invasive diagnostic and therapeutic cardiac electrophysiology) - Providers who are specialized in invasive and diagnostic and therapeutic CCEP. Examples would include the ability to perform ablations, programmed electrical stimulation, endocardial mapping, and trans-septal catheterization. Providers will have advance technical abilities and cognitive skills in management of patient with complex arrhythmia.
