Home Health LPN Intake Referral Coordinator Full Time

Position Overview:

Join our care team as an LPN Clinical Intake Coordinator and help drive high-quality, compliant, and efficient home healthcare. You'll be the go-to for clinical oversight, documentation review, and client supportensuring our standards stay top-notch while optimizing operations.

Schedule: In-office position Monday - Friday supporting normal business hours

What Youll Do

• Assess, triage, and resolve client concerns to maintain exceptional care

• Collaborate with field staff and leadership to coordinate services and review clinical documentation

• Communicate with families, physicians, and other providers to support client outcomes

• Assist in policy development and ensure adherence to regulations and protocols

• Fill in for staffing gaps and participate in on-call rotations

• Offer process improvement ideas and manage medical supplies with cost-efficiency

• Prepare case reviews and help with accreditation and payer compliance

What You Bring

• Active LPN license (state-specific)

• CPR certified

• Valid drivers license, insurance, reliable transport, clean driving record

• Available for on-call scheduling

Bonus Points

• Experience in home care

• Knowledge of Medicare and accreditation standards

• EMR savvy (HCHB preferred)

• Willing to travel as needed

• Geriatric Home Health experience preferred
