As an LPN/RN at The Center at Blue Hills, you will be responsible for providing high-quality nursing care to our residents, managing medications, observing and reporting changes in resident condition, and collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities:

• Provide direct patient care to residents, including assessments, medications, and treatments
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report changes in resident condition to the physician and other healthcare professionals
• Maintain accurate and up-to-date records of resident care and treatment
• Participate in quality improvement initiatives and maintain ongoing education and training
