Adult Day Program Coordinator

Our Adult Day Program is looking for a dependable, compassionate, and organized Adult Day Program Coordinator to help lead our program and make each day meaningful for the people we serve.

This position is a great fit for someone who enjoys working with older adults, building relationships with families, supporting staff, and having a little fun along the way. Our days are filled with activities, conversation, laughter, meals, outings, and plenty of variety.

What You’ll Do

As our Adult Day Program Coordinator, you’ll help keep the program running smoothly while making sure our participants receive safe, individualized, and engaging care. You’ll serve as the on-site supervisor for program aides and work closely with the Eldercare Director, who will provide ongoing support and oversight.

Your responsibilities will include:

• Helping with overseeing the day-to-day operations of the Adult Day Program.
• Creating a safe, welcoming, and engaging environment for participants.
• Recognizing and responding to changes in participants’ conditions.
• Administering medications and treatments according to provider orders and organizational policies.
• Providing direct supervision, guidance, and support to Adult Day Program aides.
• Communicating with families, caregivers, healthcare providers, and other members of the care team.
• Assisting with participant enrollment, assessments, care planning, and ongoing service needs.
• Completing timely and accurate documentation.
• Helping coordinate activities that support participants’ physical, social, cognitive, and emotional well-being.
• Working alongside the Eldercare Director to support quality improvement and continued program growth.
• Helping ensure the program remains compliant with applicable Minnesota requirements and organizational policies.

What We’re Looking For

Option 1:
Be a Licensed Practical Nurse (LPN) in Minnesota or have completed at least two years of postsecondary education from an accredited institution , AND have at least two years of paid or volunteer experience planning or delivering health or social services, including experience in supervision and administration .

OR

Option 2:
Have at least four years of paid or volunteer experience planning or delivering health or social services, including at least two years of experience in supervision and administration .

Additional qualifications include:

• Valid driver’s license.
• Current CPR and first aid certification, as applicable to program requirements.
• Strong leadership, communication, and organizational skills.
• Ability to supervise, support, and mentor program staff.
• Experience in adult day services, long-term care, assisted living, or community-based care is preferred.
• A compassionate, person-centered approach to working with older adults and individuals with varying care needs.
• Ability to multitask, prioritize, problem-solve, and adjust when the day doesn’t go exactly as planned.
