Registered NurseThe RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence, current research, hospital policies, procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines.Responsibilities include assessment of patient needs, performing and documenting comprehensive assessments in accordance with RN scope of practice and policies. The RN considers the patient's culture, age, and developmental level. The RN identifies patient risk factors and presenting complaints while obtaining health information from family or others as necessary. Objective and subjective data is collected, and initial admission assessments are verified if obtained by others. A problem list is developed, prioritizing patient needs based on inter-professional team input. Actions and interventions are designed relevant to the patient's culture and age, and desired outcomes are determined in coordination with the patient and family. The RN fully implements patient care therapies and procedures in accordance with established guidelines. The RN explains procedures, treatments, and processes to patients and their families. Initiatives are taken to alleviate patient stress and discomfort while recognizing, responding to, and communicating changes in patient conditions appropriately. Ongoing assessments and responses to procedures are documented according to hospital policies and established standards. The RN delegates tasks appropriately and oversees team members in patient care activities. Accuracy and completeness of delegated care are monitored in implementing the plan of care. Educational needs of patients are identified and addressed related to prioritized problems based on comprehension level. The discharge process is facilitated by executing discharge orders in a timely manner, with appropriate teaching and coordination with case management. Input into patient/nurse assignments is provided while considering available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is demonstrated. The RN promotes a positive group dynamic and healing environment through words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practices, and a caring philosophy. Participation in department and unit quality improvement activities and core measures is expected. A holistic perspective regarding patient care is emphasized.
