Inpatient Social Work Case Manager: Discharge Planning

The University of Kansas Health System is seeking a full-time Inpatient Social Work Case Manager in Kansas City. This role is responsible for coordinating care services post-hospitalization, conducting psychosocial assessments, and advocating for patients' rights and needs. A Master's in Social Work and appropriate state licensure (LSCSW or LMSW) are required. The ideal candidate should have skills in patient advocacy, care coordination, and active listening.
