Job Title: LPN – Long-Term Care

Location: Falmouth, MA
Duration: 13 Weeks
Schedule: 40 Hours/Week
Shifts: Rotating Days, Evenings & Nights
Shift Times: 3:00 PM–11:30 PM and 11:00 PM–7:30 AM
Weekend/Holiday: Every other weekend and holiday

Compensation

Travel Gross Weekly: $1,900–$2,000/week, based on experience
Local Rate: $45–$50/hour, based on experience

Position Overview

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care setting. The LPN will also supervise nursing assistants and ensure that residents receive high-quality care in accordance with applicable federal, state, and local regulations and standards.

Primary Responsibilities

• Provide direct nursing care to residents and supervise nursing assistants.
• Coordinate day-to-day nursing activities and services for assigned shifts.
• Participate in planning resident care, services, programs, and activities.
• Complete required documentation and medical records for resident admissions, transfers, and discharges.
• Receive and accurately document physician orders and telephone orders.
• Transcribe physician orders to appropriate medical records, medication records, and treatment/care plans.
• Maintain accurate, timely, and informative nursing documentation reflecting care provided and resident responses.
• Communicate information regarding new admissions, transfers, and resident needs to appropriate nursing personnel.
• Participate in regulatory surveys and inspections as required.
• Perform nursing administrative functions as assigned.
• Assist with orientation of new residents and family members.
• Make rounds with physicians as necessary.
• Communicate with physicians regarding resident care, treatment, rehabilitation, and changes in condition.
• Review resident charts for treatments, medication orders, dietary requirements, and other care needs.
• Prepare and administer medications and treatments as ordered, following appropriate resident identification and medication safety procedures.
• Ensure adequate nursing supplies, medications, and equipment are available and report shortages appropriately.
• Perform nursing procedures including catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage bag care, blood collection, range-of-motion exercises, and other appropriate clinical services.
• Collect specimens such as urine, blood, and sputum for testing and perform routine laboratory procedures as required.
• Monitor and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Monitor dietary compliance and report concerns regarding resident nutrition or dietary restrictions.
• Provide health education and instructions to residents and their families.
• Maintain strict confidentiality of resident information and protected health information.
• Uphold resident rights and report suspected unauthorized disclosure of confidential information.
• Perform other related duties as assigned.

Qualifications & Requirements

• Graduate of an approved LPN program or possess an appropriate nursing degree from an accredited institution.
• Current, active, and unencumbered Massachusetts LPN license .
• BLS/CPR and AED certification required.
• Ability to read, write, speak, and understand English.
• Knowledge and skills necessary to provide appropriate care based on residents' age-related needs.
• Strong teamwork and collaboration skills.
• Ability to make independent decisions when appropriate.
• Ability to communicate and interact professionally with residents, families, coworkers, visitors, and other members of the healthcare team.
• Knowledge of nursing and medical practices, procedures, and applicable regulations governing long-term care.
• Strong leadership and supervisory abilities.
• Ability to organize, plan, implement, and evaluate nursing care and related programs.
• Compassionate, patient, professional, and positive attitude when working with residents.
• Willingness to learn and incorporate new nursing methods and practices.
• Ability to communicate effectively regarding a resident's condition and care needs.
• Must be able to meet applicable physical and sensory requirements of the position.
• Must be able to safely lift at least 50 pounds and perform other required physical duties.
• Must not pose a direct threat to the health or safety of others in the workplace.
