What You'll Do:  

• Monitor resident wellness and respond to changes in condition as needed
• Guide and support caregiving staff to maintain quality resident services
• Partner with community leadership to coordinate resident care needs
• Complete assessments and update individualized care plans with resident and family input
• Coordinate appointments, referrals, transportation, and care-related communication
• Oversee medication-related services and ensure regulatory compliance
• Document incidents, investigate concerns, and follow through on action plans
• Collaborate with residents, families, and team members to promote safety and well-being

What You'll Need:  

• Current and active RN license in New Jersey (required) 
• 1+ years of nursing experience preferred
• Strong interpersonal and communication skills
