Role Description The Registered Nurse Case Manager is a full-time, on-site role located in Stuart, FL. This role is responsible for coordinating and delivering holistic hospice nursing care to patients and families, including comprehensive assessments, symptom and pain management, and development of individualized care plans. The Registered Nurse Case Manager collaborates with interdisciplinary team members, physicians, social workers, chaplains, and volunteers to ensure continuity of care and appropriate resource utilization. Daily responsibilities include ongoing evaluation of patient needs, discharge planning when appropriate, education and support for patients and caregivers, and accurate, timely documentation in accordance with regulatory and organizational standards. The role also involves participation in team meetings, quality improvement initiatives, and advocacy for patient preferences and goals of care. Qualifications Strong clinical Nursing and Medicine skills, with the ability to provide comprehensive assessments and hospice-level care. Demonstrated expertise in Case Management, including coordination of services and interdisciplinary care planning. Experience with Discharge Planning, ensuring safe transitions of care and appropriate community resources. Knowledge of Utilization Management principles to support effective use of hospice services and compliance with regulatory requirements. Current, unrestricted Registered Nurse (RN) license in the state of Florida. Experience in hospice, palliative care, or related acute/chronic care settings preferred. Excellent communication, critical thinking, and problem‑solving skills, with the ability to support patients and families in emotionally challenging situations. Ability to work collaboratively within an interdisciplinary team and to manage a caseload in a fast‑paced clinical environment.
