Physical Therapist (PT) - Part-TimeTampico Healthcare Center, a 128-bed skilled nursing rehabilitation center located in Walnut Creek, CA, is seeking a Part-Time Physical Therapist to join our in-house rehab team. As a Physical Therapist at Tampico Healthcare Center, you will be responsible for planning, delegating, and providing safe and competent physical therapy interventions as ordered by physicians. You will work closely with our interdisciplinary team to ensure the highest degree of quality care for our residents.Responsibilities:Provide physical therapy assessments and develop individualized treatment plans to increase clients' independence and functioningSupervise Physical Therapy Assistants, provisional licensees, students, and support personnel in accordance with state licensure requirements and professional standardsIdentify the need to make referrals to other disciplines to address comprehensive care needs of clientsDemonstrate competent technical skills according to current Physical Therapy standards of practiceCommunicate changes in residents' conditions and participate in discharge and community mobility planningPrescribe necessary durable medical equipment to facilitate independenceProvide accurate and timely billing records and supervise supporting clinical documentationObserve confidentiality and safeguards for all patient informationTraining and Education:Participate in collaborative care conferencesCoordinate and provide training with Restorative Nursing and other Care staffProvide education and training to patients, caregivers, and community partnersAssist with training and education of facility and professional partnersEnsure compliance with mandatory education, training, and competency programsParticipation in PTA/PT student development and educational programsDepartment Duties:Perform screening rounds as directed by the Director of Rehabilitation to assess for client needs and environmental safetyProvide recommendations to the Director of Rehab and other team members on opportunities for business improvement, safety concerns, or other updatesRequirements:Current physical therapy practices and procedures, including laws, regulations, and guidelines that pertain to nursing care facilitiesExperience in providing appropriate care in post-acute or long-term care environment (preferred)Must be a graduate of an accredited Physical Therapy Program by APTA CAPTECurrent, active Physical Therapy License (required)Work Environment:128-bed skilled nursing rehabilitation center located in Walnut Creek, CA, within walking distance to John Muir Medical CenterSalary: $50 - $65 HOURLYJob Type: PART TIMEMinimum Years of Experience: We Welcome New Grads!!Level of Education: BACHELORSIf you are a motivated and compassionate Physical Therapist looking to join a dynamic team, please apply to this exciting opportunity.
