Join us in improving the health and overall well-being of every resident in Tulsa County.Starting at $42996.96 AnnuallyPOSITION SUMMARY:Within Healthy Start guidelines, provides case management services to Tulsa County pregnant women, women of childbearing age, and their families in order to improve birth outcomes and decrease infant mortality.PRINCIPAL DUTIES/RESPONSIBILITIES:The following functions represent the majority of the duties performed by the position, but are not meant to be all-inclusive or prevent other duties from being assigned when necessary.ESSENTIAL JOB FUNCTIONS:Provide case management services to include screening, assessment, and education, and determine the preferred intervention strategy for each client according to Healthy Start protocols specifically within the Tulsa County zip codes of: 74063, 74011, 74012, 74106, 74110, 74115, 74116, 74117, 74126, 74127, and 74130.Work with the Fatherhood Case Manager as appropriate, maintaining regular and ongoing (at least monthly) communication to ensure effective and coordinated services to the family.Complete client documentation and ensure accurate database entry according to Healthy Start guidelines. Ensure HIPAA confidentiality, voluntary consent to participate, and as appropriate, release-of-information documentation.Recruit and maintain appropriate client caseload levels as defined by management team.Interview clients and complete intake, screening tools, and risk assessments, which identify specific needs of each client and family.Staff individual clients with Case Manager Supervisor according to staffing protocol.Make referrals to community resources; follow up on missed medical appointments; make home visits; and work with clients to identify and eliminate barriers to referral completion.Coordinate with medical and behavioral health, as well as social services to improve quality of care.Attend Healthy Start specific training and planning meetings, staff meetings, quality assurance activities, and appropriate in-service.Attend required training in support of duties and responsibilities of this position.Other duties as assigned including those required to fulfill activities in support of public health emergency operations.QUALIFICATIONS/SPECIFICATIONS:Education: Bachelor’s degree in social work or related field required; master’s degree in health or a related field preferred.Experience: Three years of experience as a case manager or related experience with Maternal-Child Health (MCH) preferred.Knowledge and Skills:Excellent interpersonal and good communication skills requiredAbility to obtain cooperation from clients who come from a wide range of economic, social, and cultural backgroundsSkills in interviewing and engaging clients; and in working with agency staff and community resourcesComfortable in gathering information about and providing education related to reproductive healthAbility to work closely and cooperatively with programmatic co-workers toward a Healthy Start family’s goalsKnowledge of community resources and helping relationships that would assist mothers, babies, and/or familiesBilingual – Spanish may be requiredLicenses:Eligible for licensure as a social worker by the state of Oklahoma if appropriateValid Oklahoma driver’s licenseINTERNAL AND EXTERNAL WORKING RELATIONSHIPS:Regular internal contact with various staff to coordinate care or referrals for patientsExternal contact with agencies to exchange information in order to coordinate care, referrals, and programs for clients; and with clients to gather information and provide counseling servicesPHYSICAL EFFORT:Ability to visit clients’ homes; conduct interviews; attend meetings; sit for extended periods of time; and carry charts to staffing meetings.SUPERVISORY RESPONSIBILITY/ACCOUNTABILITY:Direct Supervision – NoneIndirect Supervision – NoneBudget/Money/Material – Responsible for assigned equipmentReports to – Manager, Healthy Start InitiativeLocation – North Regional Health & Wellness CenterWORKING CONDITIONS:Regular exposure to a noisy and crowded work environment, and occasional interaction with sometimes disturbed and/or abusive people.SPECIAL REQUIREMENTS:Must maintain THD record confidentiality according to HIPAA regulationsMust possess ability and willingness to perform job-related travelFLSA Status: ExemptBenefits:We offer a complete benefits package to full-time employees including health care, dental, vision, retirement, deferred compensation plans, flexible spending accounts, life insurance, long-term disability, holidays, and paid time off.Part-time employees who work an average of 20 hours per week over the course of a year, receive benefits on a pro-rated basis.See below for more details or visit our benefit page Benefits | Tulsa Health Department (tulsa-health.org):Competitive SalaryPaid Holidays (typically 13 days per calendar year + 2 floating holidays)Paid Time Off (PTO) 20 hours per monthCost of Living Adjustments (COLAs)Retirement Pension Fund with a 5-year vesting period.
