In this position, you will provide one-on-one therapy services to patients in their homes throughout Atlanta and surrounding communities. You will work under the direction of a licensed Physical Therapist to implement individualized plans of care focused on improving mobility, safety, and independence. Typical patients include adults and older adults recovering from surgeries, hospitalizations, or managing chronic conditions such as stroke, cardiac disease, or orthopedic issues. A typical day includes traveling between patient residences, performing in-home therapy visits, and completing documentation via a mobile EMR platform. You will conduct therapeutic exercises, gait and balance training, functional mobility activities, and home safety interventions, while educating patients and families on safe techniques, equipment use, and strategies to support independence. Visit volumes and patient loads are structured to support quality, patient-centered care rather than rushed appointments. You will collaborate closely with the supervising Physical Therapist, nurses, occupational therapists, and other members of the interdisciplinary team to coordinate care and adjust treatment plans as needed. The role requires strong clinical judgment in the home environment, where each visit may present unique challenges related to space, home layouts, and individual patient needs. Schedules typically include daytime hours with some flexibility to accommodate patient preferences and travel considerations across the Atlanta metro area.
