As a Home Health SLP, you will oversee the assessment and treatment of patients experiencing speech, language, cognitive, and swallowing challenges:

• Conduct thorough evaluations of speech, language, cognitive, and swallowing abilities.
• Create and execute personalized care plans aimed at enhancing communication, cognition, and safety.
• Implement therapeutic interventions utilizing evidence-based methods and specialized tools.
• Evaluate the home setting and suggest adaptive equipment or communication aids as necessary.
• Educate patients and their caregivers on exercises, compensatory strategies, and safety measures.
• Track patient progress, reassess as required, and modify treatment plans accordingly.
• Identify any changes in patient condition and communicate proactively with physicians and the care team.
• Accurately document all care in a timely manner within the electronic medical record (EMR).
• Collaborate with nurses, therapists, and interdisciplinary teams to ensure cohesive care.
• Engage in case conferences, discharge planning, and ongoing care coordination.

• Master's or Doctoral degree in Speech-Language Pathology.
• Active SLP license in the state where you practice.
• At least 1 year of clinical experience (home health experience preferred).
• Certificate of Clinical Competence (CCC-SLP) is preferred.
• Strong clinical judgment with the ability to work autonomously.
• Excellent communication and patient education abilities.
• Comfortable in a field-based role with travel between patient residences.
• Current CPR certification.
• Valid driver's license, dependable transportation, and auto insurance.

• Experience in home health or community-based care settings.
• Confidence in managing swallowing (dysphagia) and cognitive-communication disorders.
• Ability to tailor treatment plans to fit real-life home situations.
