Position Summary:

The Case Manager provides individualized coordination of care for patients, ensuring access to needed services and support. This role involves comprehensive assessments, care planning, advocacy, and resource linkage.

Essential Duties and Responsibilities:

●      Conducts intake assessments to identify patient needs and goals.

●      Develops individualized service plans in collaboration with the treatment team.

●      Provides crisis intervention and emotional support.

●      Coordinates referrals to external providers, social services, housing, employment, and other resources.

●      Monitors and evaluates patient progress towards goals, updating treatment plans as needed.

●      Provides advocacy for patients navigating behavioral health and social service systems.

●      Maintains thorough and timely documentation of services delivered.

Minimum Qualifications:

●       Education: Bachelor’s degree in social work, psychology, behavioral health, or related field required.

●       Experience: At least one (1) year of behavioral health case management experience preferred.

Knowledge, Skills, and Abilities:

●      Knowledge of community resources, behavioral health systems, and case management principles.

●      Experience working with diverse populations.

●      Ability to advocate effectively on behalf of patients.

●      Advanced knowledge of mental health disorders and treatments.

●      Strong crisis intervention skills.

●      Strong documentation, organizational, and communication skills.

●      Ability to work collaboratively in a team environment.
