Registered Nurse Case Manager — Home Health
Location: Wood, Wirt & Calhoun Counties, WV
Schedule: Monday–Friday, Day Shift
Hours: 40 hours guaranteed | No on-call
Mileage: $0.49/mile
License: WV or Compact RN
Productivity: Minimum 30 points/week
We’re seeking an experienced Registered Nurse Case Manager with strong Home Health experience to support high-risk patients and coordinate comprehensive, patient-centered care.
Responsibilities
Perform comprehensive patient assessments and develop individualized care plans.
Coordinate care with physicians, social workers, discharge planners, and other healthcare professionals.
Manage care transitions and connect patients with medical and community resources.
Monitor clinical, functional, and fiscal outcomes.
Promote prevention, patient safety, and improved health outcomes.
Advocate for patients while maintaining privacy and confidentiality.
Requirements
Associate’s or Bachelor’s degree in Nursing or related field.
Current, unrestricted WV or Compact RN license.
Home Health experience required.
OASIS experience required.
HCHB experience required.
Case Management certification.
Strong clinical assessment, critical-thinking, and communication skills.
Great opportunity for an experienced Home Health RN looking for a consistent Monday–Friday schedule with no on-call requirement.
