Family Nurse Practitioner (FNP) / Physician Assistant (PA) Location: Mesa, AZ and West Pheonix, AZ Experience Required: Minimum 2 years (mandatory) Schedule Monday & Tuesday: 7:00 AM - 6:00 PM Wednesday: Off Thursday: 8:00 AM - 7:00 PM Friday: 7:00 AM - 6:00 PM Job Description We are seeking an experienced Family Nurse Practitioner (FNP) to provide comprehensive primary care services in an outpatient setting. This role focuses on delivering high-quality, patient-centered care through assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions. The ideal provider practices independently while collaborating with a multidisciplinary care team and adhering to established clinical standards and protocols. Key Responsibilities Provide primary care services to patients of all ages Assess, diagnose, and manage acute and chronic medical conditions Deliver preventive care, health screenings, and patient education Develop and implement evidence-based treatment plans Prescribe medications and order diagnostic tests as appropriate Maintain accurate and timely clinical documentation in the EHR Coordinate referrals and collaborate with other healthcare professionals Ensure compliance with clinical, regulatory, and safety standards Qualifications Graduate of an accredited Nurse Practitioner program Certified Family Nurse Practitioner (FNP) Active Arizona RN and APRN licenses Current, unrestricted DEA registration BLS / CPR & AED certification (AHA or Red Cross) Minimum 2 years of independent clinical experience Proficiency with electronic health records Strong communication and interpersonal skills Cultural competence and commitment to serving diverse populations Valid Arizona driver's license Level One Fingerprint Clearance Card (required for certain locations) Compensation & Benefits Salary: $110,000 - $135,000 (DOE) Full benefits package Supportive clinical environment Opportunity to make a meaningful impact in the community #J-18808-Ljbffr
