Home Health Case Manager II (Union S-RN)- SIGN-ON BONUS

Position Summary The Home Health Case Manager at Mercy Health Home Care and Hospice by Compassus is responsible for modeling the company values of Compassion, Integrity, and Excellence, and promoting the Compassus philosophy. The role includes upholding the Code of Ethical Conduct and fostering positive working relationships within the company and with external stakeholders. Responsibilities Maintain direct relationships with patients and their families. Ensure effective utilization of treatment resources to improve patient outcomes. Assume responsibility for a group of complex patients requiring initial assessment and ongoing monitoring. Conduct independent assessment, planning, implementation, and evaluation of patients’ physical, psychological, and sociocultural needs. Develop and monitor patients’ clinical pathways in collaboration with the healthcare team. Work closely with physicians and nursing/ancillary staff to implement the patient’s plan of care. Audit outcomes of caseload in collaboration with Quality Analysis and support services. Perform initial assessments to aid in the development and coordination of the plan of care. Coordinate patient case load and maintain tracking of patient progress. Communicate patient case information to physicians, staff, and other healthcare professionals. Participate in case conferences for ongoing coordination of patient care plans. Assist in developing, reviewing, and evaluating plans of care. Seek consultation with agency leadership for cases presenting significant variances. Arrange continuity of care and provide coverage during absences. Qualifications Associate level nursing degree in an accredited program; preference for a Bachelor of Science in Nursing. Minimum 1–2 years of nursing practice in medical/surgical, acute, critical care, or hospital care. Experience in a home health or hospice setting preferred. Skills Mathematical Skills: add, subtract, multiply, divide, compute rates, ratios, and percentages. Language Skills: read and interpret business periodicals, professional journals, technical procedures, and government regulations; write reports, correspondences, and procedural manuals; present information effectively. Excellent oral and written communication skills with diverse patient populations. Understand, read, write, and speak English. Demonstrate integrated healthcare at home philosophy and clinical competency. Use equipment essential to the care of patient age groups; integrate clinical information from multiple disciplines. Knowledge of community resources, third‑party payors, Medicare/Medicaid regulations, and federal/state rules. Certifications, Licenses, and Registrations Current RN license in the state of Ohio and CPR certification. Physical Demands and Work Environment Requires standing, walking, manual dexterity, and the ability to lift and move objects up to 50 pounds. Visual acuity for close and distance vision, color and peripheral vision, depth perception, and focus adjustment is necessary. Exposure to bodily fluids and infectious diseases is expected; routine use of standard medical equipment and tools is essential. Benefits Competitive pay with a $5,000 sign‑on bonus.
