Job Summary:

We are seeking a highly skilled and compassionate LPN Licensed Practical Nurse to join our team of dedicated healthcare professionals at Haven Place, a premier residential, long-term, rehabilitation, and subacute care provider. As a Specialty Nights LPN, you will play a vital role in delivering exceptional patient care to our residents. Responsibilities:

  • Provide direct patient care to residents, including nursing interventions, medication administration, and vital sign monitoring

  • Conduct routine assessments and develop individualized care plans to meet the unique needs of each resident

  • Collaborate with other members of the care team, including RNs, therapists, and physicians, to ensure comprehensive care

  • Educate patients and families on disease management, medication usage, and other health-related topics

  • Maintain accurate and detailed records of patient care and treatment

  • Participate in quality improvement initiatives to improve patient outcomes and satisfaction
