Nurse Practitioner

Job Summary The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in ChenMed/Jencare medical centers, nursing homes, skilled nursing facilities (SNF), and home settings, as well as providing assessments to members in SNF and home settings. Responsibilities Conduct geriatric assessments, medical history, physical examinations, diagnoses, and treatments; develop nursing plans of care; provide health education; coordinate physician and case management referrals; and maintain clear documentation according to ChenMed standards for quality, service, productivity, and teamwork. Participate in clinical rounds and conferences; produce in-depth progress notes and summaries. Collaborate with managers, physicians, and medical directors to ensure compliance with guidelines; participate in risk and quality management programs and clinical meetings. Assess and manage acute and non‑acute clinical problems independently; document physical assessments, patient histories, analyze trends, and develop and implement patient management plans. Assist in the development of nursing care plans; provide counseling and education to patients and their families. Write admission, transfer, and discharge orders; order and interpret appropriate laboratory and diagnostic studies; prescribe medications and treatments; refer patients for specialized consults as indicated (dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social services, dietary, etc.). Perform invasive procedures independently once demonstrated competency. Participate in patient care rounds and conferences; communicate management strategies to the patient care team; collaborate with multidisciplinary team members to ensure successful patient outcomes. Recognize situations requiring physician attention and initiate life‑saving procedures when necessary. Utilize advanced communication skills to resolve complex situations and improve processes and service for patients. Collaborate with multidisciplinary team members to analyze and evaluate existing health‑care delivery systems and implement new practice patterns. Engage in outside activities that enhance personal and professional growth and development. Facilitate transitions to lower levels of care, facilitate patient and family conferences, optimize resource utilization, and manage discharge plans; prescribe medication in accordance with state practice. Perform other duties as assigned and modified by the manager. Qualifications Demonstrated record of consistently achieving clinical performance metrics in current role. Strong critical thinking and problem‑solving skills. Excellent communication and interpersonal skills. Time‑management skills and ability to work well under pressure. Empathetic, caring, with strong listening skills. Detail‑oriented with close attention to patient charts, medication lists, and follow‑up details. Basic computer skills and familiarity with Microsoft Office Suite. Ability to travel locally 50–75% of the time. Compensation $111,140 – $158,771 base salary or hourly rate; final compensation will depend on experience, education, geographic location, and other factors.
