Medical Social Worker

Good Samaritan Hospice Indiana - Illinois is a full‑time medical social worker position. Hours are 5 days, 8:00 AM–5:00 PM (no after‑hour on‑call). Job Summary and Responsibilities We are a hospice organization serving Southwestern Indiana (Davies, Gibson, Greene, Kox, Martin, Pike, Sullivan) and Illinois (Lawrence, Wabash, Crawford, Richland). As a Medical Social Worker you will: Establish a care plan in accordance with physician's plan of treatment. Provide crisis intervention as required. Assist the physician and caregivers in understanding significant social, emotional, and economic factors influencing the patient's recovery. Offer patient and caregiver education. Make appropriate community referrals for additional resources (public assistance, housing, healthcare, transportation). Work with caregivers to resolve barriers to optimal caring relationships. Complete and submit all required documentation accurately and timely. Job Requirements Master's degree from an accredited School of Social Work approved by the Council on Social Work Education. In Indiana, must also be certified under IC25‑23.6‑4‑3. Currently licensed to practice in the state served. One year social‑work experience in a healthcare setting. Current CPR/BLS certification through the American Heart Association or American Red Cross, and compliance with state requirements. Ready to Make a Difference?
