Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: 2 years of Hospice Home Health experience required License: NJ State License or Compact Certifications: BLS Required Must-Have: Must have proof of both COVID vaccines and booster at submission. Description: The position involves providing hospice community nursing and a small case load. The role requires home health hospice floating or admissions. Candidates must have experience in hospice home health. Candidates will be responsible for patient assessments, care management, and communication with interdisciplinary teams. Required competencies include pain assessment, medication management, and patient/family education and support. Knowledge of Medicare state regulations is essential. Traveling for patient visits is a component of the job, and mileage reimbursement will be provided according to IRS standards. Clinical requirements include proficiency in systematic EKG rhythm interpretation and heart rate calculations. The ideal candidate will have skills in assessing spiritual needs and providing end-of-life care. The patient demographic includes adults and geriatrics with diagnoses such as end-stage cancer and cardiac conditions. Functional skills include managing pain interventions and grief counseling during the perideath period. Floating to other units may be required as needed.
