We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full Time Physical Therapist (PT) in Gilroy. Position Overview We are seeking an experienced, self-driven Full Time Physical Therapist (PT) to join our dedicated Home Health team and provide exceptional care throughout the region. As a Physical Therapist (PT), you will evaluate patients’ health, create and implement care plans, and track their progress to maintain the highest standards of care. You will work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field. Responsibilities Provide, coordinate and oversee skilled intermittent care to patients, including instruction and education of patients and/or caregivers to promote maximal functional status, in the patient’s home. Regularly evaluate and re‑evaluate patient status. Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, and/or Director of Nursing as needed. Initiate, develop, implement and revise the patient’s plan of care (POC). Address all problems identified in the POC. Provide educational information to the patient/family/caregiver regarding knowledge deficits or needs. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation per company policy. Comply with all company policies and procedures to ensure high standards of care. Requirements Current PT license to practice in California (required). Experience in Home Health or Hospice care – 1 year (required). Current California driver’s license. Able to travel within a 15–25 mile radius on a regular basis.
