You will work collaboratively with an interdisciplinary team to deliver high-quality, patient-centered care.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plansProvide therapeutic interventions to improve strength, balance, mobility, and functionMonitor patient progress and adjust treatment plans as neededMaintain accurate and timely clinical documentationCommunicate with physicians, families, and care team membersParticipate in care planning, discharge planning, and rehab meetingsSupervise PTAs, therapy aides, and studentsProvide in-service training and education for staffQualificationsDegree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)Active, unencumbered Physical Therapist license in South Carolina (required)Experience in a skilled nursing or rehab setting preferredFamiliarity with PointClickCare (PCC) and Casamba preferredStrong clinical, leadership, and communication skillsCommitment to continuing education and professional developmentWhy Join Reedy River Post Acute?At Reedy River, you’re part of a team that values your expertise and invests in your success.
