Responsibilities

• Conduct comprehensive speech, language, voice, and cognitive-communication assessments for patients in an acute care and SNF setting.
• Develop and implement individualized treatment plans targeting communication and swallowing disorders.
• Provide direct therapy services to patients with dysphagia, aphasia, apraxia, dysarthria, and related conditions; use evidence-based methods.
• Document evaluations, progress notes, treatment goals, and outcome measures; ensure compliance with privacy and coding standards.
• Collaborate with an interdisciplinary team (nursing, physicians, OT/PT, dietitian) to coordinate care, discharge planning, and safety considerations.
• Educate patients, families, and caregivers on strategies to support communication and safe swallowing; train staff as needed.

Requirements

• Active Speech-Language Pathology license in Pennsylvania (or eligibility for PA licensure)
• Ability to work in a skilled nursing facility setting and participate in an interdisciplinary team
• Strong assessment, planning, and documentation skills
• Excellent communication and collaboration abilities with patients, families, and colleagues
• Comfort with travel-based assignments and adherence to shift schedules and facility policies
