Perform targeted physical therapy interventions aiming to enhance mobility, strength, balance, and overall function

Partner with nurses, physicians, case managers, and an array of rehab professionals to maximize patient outcomes

Guide patients and their families by providing essential knowledge about therapeutic goals, discharge plans, and at-home exercises

Maintain accurate documentation of evaluations, treatments, and progress in line with facility protocols and regulatory guidelines

Engage actively in care team discussions and interdisciplinary meetings to ensure cohesive and coordinated patient care

Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

State licensure as a Physical Therapist

CPR certification is strongly preferred

Additional specialized certifications (neurological or geriatric focus) are advantageous but not mandatory

Prior experience preferred, especially in settings with diverse patient needs
