Hospital Social Worker | Crisis & Discharge Planning
Memorial Hermann Health System
Memorial Hermann Health System is hiring a Licensed Master Social Worker to provide clinical social work and complex discharge planning for patients with psychosocial needs. You will work within an interdisciplinary team to develop discharge plans and connect patients and families with community resources. The role reports to the Manager or Director of Case Management.
