This is a full-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.ResponsibilitiesProvide primary care services to residents in skilled nursing and long-term care facilitiesPerform physical exams and medical history assessmentsDiagnose and treat chronic and acute health conditionsOrder and interpret diagnostic tests and lab resultsDevelop and adjust individualized care plansPrescribe medications and monitor treatment effectivenessCollaborate with facility staff and interdisciplinary teamsEducate patients and families about treatment options and care goalsEnsure a safe, respectful, and supportive environment for residentsRequirementsActive and unrestricted Nurse Practitioner, Physician Assistant, or Physician licenseState-specific license in good standing in New YorkMinimum 6 months of experience in long-term care or a skilled nursing facilityStrong communication, clinical, and critical thinking skillsPassion for elder care and person-centered healthcareProlonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients.
