Client Care Coordinator

Own the day-to-day client experience for South West Georgia Homecare. You will run intakes, match caregivers to families, build the schedule, and be the named person families can reach directly. Responsibilities Complete client intakes and in-home service assessments Match caregivers to clients by skills, personality, and proximity Build and maintain the weekly caregiver schedule Serve as the named point of contact for families Coordinate hospital discharge and referral requests Document service changes and escalation condition changes same day Requirements Two or more years of scheduling, care coordination, or healthcare operations experience Excellent phone and written communication with families Comfort working in scheduling and electronic record systems Strong organization under shifting daily priorities Knowledge of Dougherty, Lee, and surrounding counties preferred Ability to pass background screening #J-18808-Ljbffr SWGA Homecare
